INTERNAL WHISTLEBLOWER REPORT E-FORM – التقرير الالكتروني للإبلاغ عن المخالفات الداخلية

    The Regulatory Authority (“RA”) is committed to maintaining an ethical environment and takes any wrongdoing by its officers or employees, whether by act or omission seriously. All employees are encouraged to report any form of wrongdoing, which in good faith they reasonably believe has occurred or will take place, with the RA’s guarantee that such concerns will be treated seriously and investigated properly. If specifically requested, the identity of an employee who files a report will be protected to the greatest extent possible and a zero tolerance approach is taken against any threat of retaliation.
    The form below is provided for your convenience to be able to file a report anonymously or not per your wishes pursuant to the RA Whistleblower Policy, available for your review on the Intranet. We ask that this form be used only for matters relating to the Whistleblower Policy and not grievances that fall under other policies and procedures. Please fill out the boxes to the best of your ability and upload as much supporting documentation as possible. This form is accessible via the RA website so you do not need Intranet access or to be in the office to file a report.

    تلتزم هيئة تنظيم مركز قطر للمال بالحفاظ على بيئة أخلاقية وأن تنظر بجدية في أي مخالفات من قبل مسؤوليها أو موظفيها، سواء كانت عن طرق القيام بعمل أو الامتناع عن القيام بعمل. يتم تشجيع جميع الموظفين على الإبلاغ عن أي شكل من أشكال المخالفات ، التي يعتقدون بحسن نية أنها حدثت أو ستحدث ، مع ضمان الهيئة بأن يتم التعامل مع هذه المخاوف بجدية واجراء التحقيقات اللازمة . ستتم حماية هوية الموظف الذي يقدم تقريرًا إلى أقصى حد ممكن إذا طلب الموظف عدم الكشف عن هويته
    .وستقوم هيئة التنظيم باتخاذ اجراءات حازمة اذا تعرض مقدم الشكوى لأي نوع من أنواع التهديد او الانتقام
    تم توفير النموذج أدناه لتتمكن من تقديم تقرير سواء بسرية تامة أو بشكل معلن بحسب رغبتك وفقًا لسياسة الإبلاغ عن المخالفات لدى هيئة التظيم، والتي يمكن الإطلاع عليها في الموقع الخاص بهيئة التنظيم . يرجى أن يتم استخدام هذا النموذج فقط في الأمور
    .المتعلقة بسياسة الإبلاغ عن المخالفات وليس المظالم التي تندرج تحت السياسات والإجراءات الأخرى. يرجى تعبئة هذا النموذج وفقاً لما هو مطلوب وتحميل أكبر قدر ممكن من الوثائق الداعمة
